The middle cardiac vein is a relatively large vein that commences near to the apex of the heart on its inferior surface. It passes superiorly and to the right in the posterior part of the interventricular groove in the company of the posterior interventricular artery. At the junction of interventricular and atrioventricular grooves on the posterior surface of the heart, it drains into the coronary sinus.
The middle cardiac vein drains part of the right and left ventricles.
