An anal fissure is seen as a crack in the anal canal, usually directly posteriorly and in the midline. Pain may prohibit rectal examination if the fissure has developed acutely. Chronic fissures may be distinguished by the presence of a sentinel pile, a tag of skin at the fissure base.
