A primary or true scrotal swelling is one that an examiner can get above. It may be cystic or solid.

Cystic primary scrotal swellings include:

  • those that can be felt separate from the testis:
    • epididymal cysts
    • spermatocoele
  • those in which the testis lies within the swelling:
    • hydrocoele
    • haematocoele - does not transilluminate

Solid primary scrotal masses include:

  • those that can be felt separate from the testis:
    • chronic epididymitis
    • acute epididymitis
    • torsion of hydatid of Morgagni
  • those are within the testis:
    • testicular tumour
    • torsion of the testis
    • orchitis
    • testicular gumma
