The differential diagnosis of aplastic anaemia is effectively, exclusion of other causes of pancytopenia:

  • other bone marrow disorders:
    • myelodysplasia
    • acute leukaemia
    • myelofibrosis
    • infiltrative disease - lymphoma, myeloma, carcinoma, hairy cell leukaemia
    • megaloblastic anaemia

  • non-marrow disorders:
    • hypersplenism
    • systemic lupus erythematous
    • infection - tuberculosis, AIDS, brucellosis, leishmaniasis
