Diagnosis is important because the type of injury affects treatment.

Diagnosis, as always, is made on the basis of history (indicating the mechanism of injury), clinical features and special investigations (x-ray and CT features).

X-rays: Five views should be obtained;

  • AP view
  • inlet view
  • outlet view

In addition a CT scan will allow assessment of sacroiliac disruption.

When diagnosing pelvic fractures one must have four questions in ones mind;

  • What is the nature and severity of the anterior injury ?
  • What is the nature and severity of the posterior injury ?
  • What was the mechanism of injury ? AP compression, lateral compression or vertical shear.
  • Is the injury stable ?
